Here is a PDF I whipped up with forms.  You can fill it in before you run, so you don't have to decipher your own chicken scratch.  The boxes labeled 1-10 are for tracking HP of multiple creatures of the same type.

Coincidentally, I've been working up an encounter matrix pad for AD&D (which will appear for NTX I imagine) :D

My original was based on AD&D.  I modded it for AS&SH.
